International applicants living OUTSIDE the United States

Applicants wishing to take a summer online course only need to provide a copy of their passport (biographical page only). 

  1. Copy of passport (biographical page only)
  2. Proof of financial resources (i.e. bank statement or letter from a sponsor)
  3. Address where you would like to receive your immigration documents from Notre Dame and contact phone number
  4. City of birth
  5. TOEFL score report, might not be required if participating in a program (i.e. research)

International applicants currently living INSIDE the United States

Applicants wishing to take a summer online course only need to provide a copy of their passport (biographical page only). 

  1. Copy of passport (biographical page only)
  2. Most recent/current I-20 or DS-2019
  3. Most recent/current visa
  4. Employment authorization document issued for OPT, if applicable
  5. City of birth
  6. ND - International Student Questionnaire
    • Please note, if you currently hold a visa status of F-1 or J-1 sponsored by your current institution, you do not need to have the bottom of this form filled out and you will not be transferring your SEVIS record to Notre Dame. The form is still required with the top portion completed.
